Occipitocervical fixation for trauma with anterior longitudinal ligament disruption and linearized ligamentous disruption.
OBJECTIVE: It is controversial whether the unstable types of cervical spine injuries requiring stabilization should be treated by occipitocervical fixation or posterior fusion alone. A review of the literature indicates that the former method is superior. METHODS: In this study, 37 patients with a type III or IV injury were treated with occipitocervical fixation and fusion, and the results of treatment were analyzed. RESULTS: An occipitocervical fixation was performed in the following patients. RESULTS: An occipitocervical fixation was performed in 12 patients with unstable injury types III and IV. In the other 25 patients, the anterior longitudinal ligament was stabilized with wire and screws in the stable region, followed by anterior cervical discectomy and fusion. Sixteen of the 37 patients (43%) achieved radiographic bone union with occipitocervical fixation. However, in the 25 patients who had anterior longitudinal ligament instability, the fusion rate was only 38%. Sixteen of the 37 patients (43%) achieved radiographic bone union with occipitocervical fixation, but in the 25 patients who had anterior longitudinal ligament instability, the fusion rate was only 38%.About
